Templates

Labels for Centrifuge Sample Tubes: The label templates are for use on the centrifuge tubes containing either diluted ejaculate or water ballast. The labels are formatted to print on standard Avery 1" x 2.63" address label sheets (style 5160 or 5260).

The label templates are available in three forms:

• Sheet of combined of 21 Dose Information Labels and 9 Water Ballast Labels (MS Word or PDF)
• Sheet of 30 Dose Information Labels (MS Word or PDF)
• Sheet of 30 Water Ballast Labels (MS Word or PDF)
